Without a doubt, the single rarest Warzone 2 skin is the "Collision" Operator skin for Horangi, reserved exclusively for players who participated in the Call of Duty: Modern Warfare 2 open beta. Because that beta has passed, new players have zero chance to acquire this skin.

I estimate that less than 5% of the current Warzone 2 community possesses the Collision Horangi skin, making it by far the rarest cosmetic reward in the game so far.

One Shining Chance: One Weekend for Glory

Collision Horangi was only available from September 16-26, 2022. Players had to grind out XP during a single weekend to unlock the skin as a beta test reward.
